You can actually just fold a piece of paper in the way shown (which is not unique; I learned it about 18 months ago), and write shit on it.

I like it that their own folding instructions are plain wrong. 1. Start with front page at top right. This under a drawing were the front page is drawn at top left... /oops, I did it again /stops correcting people I wish I could instruct my printer to print my whole addressbook like this...

For myself, I've started printing out basic ruled PocketMod notebooks and I'm using them as bookmarks for whatever I happen to be reading at the moment. Thin enough to fit easily between the pages of your book and great for scribbling down interesting quotes, words you want to look up when you're close to a dictionary, or ideas completely unrelated to the book at hand.
